Job at a Glance: Care Coordinator
As a Care Coordinator, you are responsible for the will work within the context of medical home, from a team approach and in continuous partnership with patients, providers, and clinic staff to promote timely access of needed services and continuity of care, which will result in enhancing the patient’s health and well-being. This position requires cultural sensitivity, effective communication, proactive management of needed services and the implementation of critical thinking skills to plan and coordinate care for patients.
A minimum of 1 year of experience in a healthcare setting along with completion of a Medical Assistant or LVN program. Experience with clinic billing and ambulatory clinical settings. Bilingual English/Spanish is highly preferred.
