Daybreak, a not-for-profit organization providing street outreach services, shelter, housing and employment opportunities for homeless youth and young adults, seeks a full-time Case Manager with dynamic energy and demonstrated experience empowering high risk youth to achieve their goals and well-being.
The successful candidate will provide client care by assessing needs and developing and monitoring the client’s Individual Service plan and progress. Facilitates Daybreak’s interdisciplinary services and other community resources available to stabilize and house the youth. May accompany youth to appointments and participate in canvasing activities. Must be able to meet productivity standards set by the agency in a timely manner and participate as a member of a multi-disciplinary service team.
Bachelor's Degree in Human Services, social work, counseling, or related field and 3 years’ experienceworking with at-risk population required. LSW/LPC licensure preferred.
