Functions collaboratively within the team by observing the patient /family needs and facilitating care. The LPN provides direct nursing care to home care patients within the scope of practice for LPNs. Follows the Plan of Care (POC) established by the RN and the Interdisciplinary Team for patients.
.Planning: At subsequent visits to patient and family, follows established plan of care (POC) developed by the RN and understands the process for changing the POC according to the patient and family needs.
Implementation and coordination of services: Follows the appropriate preventative and palliative nursing procedures according to the Nurse Practice Act for LPNs.
Improvement of care: Reviews the POC with the RN and RN Team Manager and updates or revises the POC under the direction of the RN to address current patient/family needs. Participates in performance improvement activities as requested.
Performs duties in accordance with Penn Medicine and entity values, policies, and procedures • Other duties as assigned to support the unit, department, entity, and health system organization
Graduate of NLN accredited licensed practice nursing school
Licensed practical nurse for Commonwealth of Pennsylvania LPN
Current CPR certification
2+ years acute care, home or hospice care or long-term care full time experience as a licensed practical nurse.
Must be able to satisfactorily complete the orientation process. Must pass all competency testing.
0-1 years Must be proficient in reading maps/the use of GPS technology and the operation of clinical equipment/machines and computers/smart phones found in patient care setting.
Demonstrated knowledge of palliative care and home health nursing care.
Must be able to communicate to families and patients and reinforce care principles.
Contact with chemical biological, radiological materials, noise and other materials, substances or particulates that may require the use of personal protective equipment
Have a reliable transportation for daily home visits and period office meetings. Must have agency-required liability insurance.
This position is required to work every other weekend.
