We are currently seeking a full-time RN Care Coordinator at our partner hospital, Columbia St. Mary’s Hospital – Milwaukee located on Lake Drive in Milwaukee, WI to assess, develop care plans and work with hospital case management to coordinate the admission of patients to Horizon’s home care services.
We need a nurse with organizational and interpersonal communication skills to join our fun, professional and dynamic Intake team. Enjoy a salaried position working desirable 1st shift hours Monday-Friday with NO ON-CALL AND NO WEEKENDS that will challenge you and allow you to really make a difference in our patients lives every day.
Qualifications include a degree from an Accredited School of Nursing (BSN preferred), a WI RN license, at least 3-5 years of related experience, and computer/typing proficiency. Home Care experience required.
