This Advanced Practitioner will work with patients who are post heart transplant. The AP is responsible for caring for this population in both the acute and chronic settings. In this role, you would work within a multidisciplinary team and practice as independent providers in the clinic, and as consultants in the acute care management. This is an excellent opportunity for a Provider to provide longitudinal care to a complex population and be a well respected member of the team. This is a 4 day week schedule and rotation within inpatient/outpatient, and share on-call responsibilities. There is a potential for this service to go to a 5 day/week schedule.
Licensed to practice in the Commonwealth of Pennsylvania, M.S.N., and certification as an Acute Care nurse practitioner or Masters Physician Assistant. Prior to employment, the AP must be reviewed by the Credentials Committee and approved by the Medical Board.
