SUMMARY OF JOB:
Supports the Lead Admission Liaison in regards to all aspects of the Short Stay programs for Holy Redeemer Lafayette, St Joseph Manor and Transitional Care Unit.Meets life care's financial goals related to percentage of payor mix by assessing for the appropriate patients, assuring/obtaining insurance approval and maintaining occupancy for greater than 100 short stay beds.This will include assessing and evaluating patients/residents as needed prior to their admission to the facilities.Makes independent decisions regarding patient's appropriateness for which level of care within Life Care. Admission Liaison will need to develop new and foster ongoing communication and collaboration among physicians, internal and external facility staff, patients and caregivers.

LPN/ RN and/or Bachelor Degree in Business with emphasis in healthcare and experience of five years
Minimum of three years knowledge of short stay /rehab requirements for long term care setting
Experience with computer systems required, including web based applications and Microsoft Office applications which may include, Outlook, Word, Excel and Power Point
Knowledge of medical terminology and strong clinical assessment skills
Strong knowledge base of insurance companies and coverage requirements
Computer literate/Flexible hours
Ability to work independently and make decisions in a fast-paced, detail oriented manner
Demonstrates effective communication, problem-solving and critical thinking skills
