Since 1977, we have been a grassroots network of locally-focused professionals dedicated to improving the lives of our clients and community. At West Central, we provide adult case management services for our clients in both the home setting and in their community. As an Adult Case Manager you will work closely with our psychiatrists and therapists in the development of treatment plans which assist the client in cultivating life management skills. You will also help facilitate collaboration with other local agencies to coordinate services that will enhance a comprehensive treatment approach. You will directly impact the lives of adults living with mental illness through case management— development of treatment plans, skills training, psycho-educations and ongoing professional support. As an Adult Case Manager you will not only be helping your client, but also their family, and your community as a whole.

We currently have a position available in Claremont, New Hampshire
Position Requirements include:
- BA/BS degree
- 2 years of experience, preferably in a community mental health environment
- Case management experience preferred
- Excellent organizational, communication and customer service skills
- Possess working knowledge of computers and Microsoft Office
- Valid driver’s license, plus reliable and insured vehicle
