Due to our continued expanded services we offer several opportunities to join our team in a Behavioral Health Provider role.
This position provides an opportunity to perform a variety of responsibilities in various Five Rivers Health Center locations. The Behavioral Health Provider delivers clinical mental health services in a primary care setting to adults, adolescents, and their families. The Behavioral Health Provider will provide face-to-face screenings and brief interventions as well as follow up clinical services to patients. He/she will collaborate with referral sources, internal team members and educational service providers to ensure appropriate patient care and continuity of care.
The Behavioral Health Provider offers transition planning and counseling to patients and their families in an outpatient setting. He/she assesses the patient’s social, emotional, financial, cultural and family situation as needed to make referrals.
Bachelor’s Degree in relevant social science field required. Licensure with the State of Ohio as Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) required. Training and experience in the types of services provided, with age appropriate competency, using the Diagnostic and Statistical manual of Mental Disorders (DSM-IV or subsequent revisions) required.
