JOB SUMMARY: The care coordinator is responsible for coordination and management of the hospital based population including inpatient psychiatry. As a leader of the multidisciplinary care management team, the care coordinator is accountable and responsible for coordinating care and service of a selected patient population, promoting effective utilization and monitoring of health care resources, providing guidance to the interdisciplinary team to achieve desired clinical, financial and resource outcomes. Provides employee assistance, consultation to staff, physicians, and community providers regarding psychosocial aspects of care. Excellent communication skills, ability to problem solve required.

MSW with acute healthcare setting experience preferred, MA in related field acceptable with acute healthcare experience
3-5 years of clinical experience.Fulfills annual hospital mandatory requirements for safety, MSDS, infection control.Demonstrates leadership qualities.
Function independently within the broad scope of the Care Management Department’s policies and practices; generally refers specific problems to the Director of Care Management only where clarification of departmental policies and procedures are required.Requires routine knowledge of PC system including access, excel and Meditech.
