Under directand constant supervision by Nursing Supervisor, provides direct nursingservices to patients (adults and children) in free, community-based primaryhealth care clinic. Performs care coordination to high-risk high-utilizationpatients using multidisciplinary team approach.
Preparepatients for medical examination including taking medical history, performinghealth screening tests, lab tests, giving injections and emergency procedureswhen necessary. Provide healtheducation and health assessment for diabetic and pre-natal patients, as well asother special populations including high-risk high utilization patients. Assist withdispensing of medications, as needed. Triage medicalproblems over the phone and in person. Provide nursing consultation and serve as a resourcefor staff, students and volunteers. Identify unmetpatient needs and coordination with PCP to assure that standards of care aremet and gaps or barriers to care are addressed. Performpatient outreach to engage and motivate patients in self-care – interact withpatients by phone, in clinic and satellite settings, and at patients’ locationof residence as needed. Coordinatecare across the continuing including collaboration with assigned Home HealthAgencies. Complete standardized patient assessments, composeHealth Action Plans, participate in programmatic reporting. -Informally superviseunlicensed staff members. Supervisevolunteer student RN’s, medical students, clinic assistants and othervolunteers. Serve as a clinicalresource to unlicensed staff involved in care coordination functions. Develop clinicnursing procedures, as needed. Maintainnursing standards to meet licensing requirements. Involvement with quality assurance activities. Other duties as assigned within Venice Family Clinic.

BSN or MSN required
Current, active California Registered Nurse License required
Outpatient and primary nursing skills required.
Ability to perform historical, physical and behavioral status assessments required.
Ability to work as a multi-disciplinary team required.
2 years or greater of RN experience strongly preferred.
