The Community Health Paramedic’s role is to respond to Oak Street Health patient urgent care needs, as directed by the Senior Medical Director of the Community Health initiative or another member of the Indianapolis regional medical team. This position manages, supervises and is responsible for all aspects of patient’s urgent in-home care by providing an array of efficient urgent care and social support services. This position provides limited urgent care at the basic and advanced life support levels according to Oak Street Health practices, policies, and established protocols with the goal of keeping our patients well and at home.
Provides patients with high quality “out of hospital” care based upon sound science and academic research while continuously improving quality with accountability and financial sustainment
Provides specific medical care in conjunction with physician, Oak Street Health medical team
Assists with the patient and family education regarding specific disease processes and appropriate care techniques
Communicates with appropriate Oak Street Health staff regarding patient’s condition, history, treatment and response to treatments in person and/or via communication system
Carries out any additional treatment orders by physician
Takes patient histories and performs physical examinations
Participates actively in continuing education
Follows detailed or general instructions, verbal and written legibly, accurately, and confidentially uses and records data
Collaborates with other health care providers on various multidisciplinary patient care plans
Tracks the progress of patient encounters, and advocates for each patient
Coordinates and collaborates with other care providers on multidisciplinary approaches to care of these high-risk patients
Performs other duties as assigned
What are we looking for?
We’re looking for motivated, experienced paramedic with the following certifications & experience:
Ability to provide compassionate advanced care to high risk patient populations
Ability to problem solve quickly with limited information
Excellent verbal communication skills
Access to a vehicle daily is required
Must hold and maintain an up to date Paramedic license for the state hired
Advanced Cardiac Life Support Certification
American Heart Association CPR Certification
Continuing education to maintain required certifications
Practical skills, theory and medical terminology involved in basic and advanced emergency medical care
High school diploma or equivalent (GED)
Five or more years of work within an urban EMS system preferred
Make independent and critical patient care decisions
Prior hospital healthcare experience
US work authorization
