The Division Manager of Case Management fosters effective communications amongst the division, hospitals, and all disciplines. The manager champions HCA strategy and company/division/facility initiatives to ensure short and long-term objectives operationalized within North Texas Division hospitals. The Manager will identify opportunities for improvement and facilitate process standardization to promote improved outcomes, maximize resource utilization, and improve organizational efficiencies. This position is required to travel to and from facilities of North Texas Division geographic area.
Provides guidance to and actively participates with facility staff to identify, implement or enhance Case Management Care Coordination, Discharge Planning and Resource Management, consistent with HCA and facility Strategy.
Assesses Case Management performance in North Texas facilities.
Disseminates information accurately and timely with multiple audiences, ability to present to senior team leaders at the hospital and division level as requested with involvement in measurement outcomes.
Identifies process improvement opportunities; develops, implements, monitors, and revises action plans
Identifies and shares best-demonstrated practices; stays abreast of current practice standards & CMS regulatory requirements.
Assist with the development, revision and implementation of Case Management policy and procedures.
Facilitates the implementation of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives related to Case Management services; validates data integrity of required and approved regulatory reporting.
Fosters communication and facilitates collaboration between the facility Case Management, and other departments within HCA.
Facilitates Case Management education within the division; conduct orientation and on-going education of Case Management staff, provides education related to Case Coordination, Discharge Planning, Resource Management, regulatory and compliance issues.
Data analysis related to Case Management metrics; develops, implements, and monitors action plans related to Avoidable Days, LOS, and other identified case management metrics.
Performs random open chart audits to validate policy, procedure and standards are met; works with facility Director to develop action plans to address areas of opportunity.
Ensures that standards of case management practice and integrated services are developed, implemented and monitored that is consistent with professional standards and regulatory requirements.
Assesses learning needs related to case management practice and implements appropriate educational activities to expand knowledge base.
