The Early Intervention Program is seeking a Primary Service Coordinator II to plan and implement the delivery of Early Intervention services to eligible children and their families from intake through transition according to RI Operational Standards and Individual Family Service Plans. Provide intervention services to infants, toddlers and their families so that they can begin to master skills in order to begin working on overcoming limitations that result from disabilities or developmental delays. Services are provided in collaboration with families, team members and other providers that reflect the priorities of the family in a manner that is developmentally appropriate and incorporates goals and strategies into daily home routines. These services are provided in the home, community and at the agency. Will provide role-modeling and education on various topics related to parent-child development.
Functions as a member of the Early Intervention team to provide evaluation and assessment in determining eligibility and documenting progress toward goals; promotes excellence in the field and adheres to agency professional standards of care in order to continuously improve the quality of service delivery.
Education and Licensing Requirements: Bachelor's Degree in Early Childhood Education; certified by DOE in ECE OR Master’s degree in Human Services discipline (such as Special Ed, Social Work, Speech Pathology, Pediatric RN, Occupational Therapy).
