The Geriatric Nurse Practitioner is responsible for treating patients suffering from illnesses and functional decline related to aging. The Nurse Practitioner is instrumental in helping aging patients and their loved ones to navigate complex healthcare and social resources and provide emotional support to caregivers and help facilitate patient and family decision making regarding long term care and end of life care. The Nurse Practitioner works according to Maine State Rules and Regulations following established Policies and Procedures as required and within established scope of practice. The Nurse Practitioner requires skill in coordinating services, comprehensive physical and mental assessments and compassion Must be a friendly, energetic and team oriented person looking for a long term commitment. Proficient use of an EMR system, excellent communications skills and pediatric experience a plus. The Nurse Practitioner performs in accordance with CMH'S Policies and Procedures, and HIPAA.
Serves as the champion of the patient experience
Demonstrates the ability to perform physical exams on elderly and individuals with functional decline.
Obtains and documents patient's medical history. Identifies health risk factors.
Demonstrates the ability to assess, diagnose and treat acute and chronic illnesses.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Orders and interprets diagnostic tests, procedures, i.e., laboratory work, x-rays, EKG.
Demonstrates the ability to prescribe medications and non-pharmacological therapies based on the patient's diagnoses, health history and age.
Applies standardized care guidelines in clinical practice.
Provides patient and/or family with health education and counseling and referrals to appropriate healthcare professionals and community resources.
Assesses and documents patient and/or family psychosocial issues.
Treats patients and families with respect and dignity.
Collaborates with physicians and other members of the health team to plan optimal care for the patient.
Works with the patient and family to formulate a patient treatment and education plan.
Provides follow-up care, determines effectiveness of the treatment plan, reassesses and changes the plan as needed.
Documents according to standards and policies. Documentation includes all findings, interventions and results.
Functions as a patient and family advocate.
Consults with physicians and/or other health care providers when appropriate.
Coordinates patient care and makes referrals as indicated.
Demonstrates the ability to be flexible, organized and function under stressful situations.
Participates in performance improvement and continuous quality improvement (CQI) activities.
Stays current with medical advancements, new technology, new drugs through continuing education classes, society meetings, professional journals etc.
Follows the five medication rights and reduces the potential for medication errors.
Attends required departmental, organizational and medical staff committee meetings as appropriate.
Assists with development and implementation of specific policies and procedures.
Customer Service: Interacts with all individuals in a consistent manner, providing attention, support, and assistance to foster an environment of exceptional personal service.
Education and experience
Current Registered Nurse and Advanced Practice Nurse License in the State of Maine. Master's Degree in Nursing from an accredited program. Master's Degree in Nursing from an accredited program
National Certification as a Nurse Practitioner or certification eligible
Minimum of two years' experience in primary care, either family medicine or internal medicine
Current American Heart Association Healthcare Provider BLS required
Current ACLS Certification preferred
Current DEA with no restriction
Strong computer skills, including EMR/EHR
Knowledge, skills and abilities:
- Willingness to learn and work collaboratively with Geriatric and Family Physicians in the long term care and skilled nursing setting, and be passionate about care for the elderly and those at end of life
Ability to establish and maintain effective working relationships with patients and clinic staff
