To ensure smooth and fast placement at one of our dedicated Nursing homes we require you to bring with you the following documents: 

• Up-to-date Resume
• 2 legal IDs: Driver License, Passport, Permanent Resident Card or work eligibility IDs
• Social Security Card
• NY State License and Registration Certificates
• Two reference letters
• Physical Health exam results taken within the last 6 months with 1-step PPD
• Measles, Mumps, Rubella and Varicella (MMRV) lab works in the last 6 months
• Hep-B or option to sign our declination form
• Flu shot or option to sign our declination form
• Drug Screen in the last 12 months
• Covid Test
• Course Completion Certificate/Diploma
• Voided Check/ Direct deposit form for payroll

Job Qualification

• Minimal experience in Nursing Home or Hospital
• Flexible Availability
• Full-Time or Part-Time
