Due to our continued growth, we are seeking a compassionate and experienced Hospice Nurse Practitioner who will be responsible for the implementation of nursing goals, objective and evaluations.. As a Hospice Nurse Practitioner you will be participating in development of clinical policies and procedures; communicating with referral sources and physicians and collaborating with Hospice care interdisciplinary teams to provide outpatient services.
Additional duties for this Hospice Nurse Practitioner role include, but are not limited to:
Participating in the development and implementation of nursing goals, objectives and evaluation.
Collaborating with hospital, residential facilities and nursing homes nursing staffs and physicians in providing outpatient palliative care services.
- Participating in development of clinical policies and procedures.
Making decisions regarding the appropriateness of Hospice care services based on criteria for admission into the service; performing face to face visits as required and reporting back to Hospice Medical Director on assessment findings.
Promoting communication between staff, providing for and conducting staff meetings for information and staff support in conjunction with clinical program manager.
Participating in the interdisciplinary team meetings with internal and external network partners for the development and review of patient/family plan of care, including exchange of information, review of problems or unique issues, and
sharing professional responsibilities when possible.
Participating in the quality assurance processes and using continuous quality assurance philosophy and processes for the continual improvement of the agency's services.
Assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards in conjunction with the program manager.
Maintaining an effective relationship with referring physicians and oncologists and their staff.
We are looking for a Hospice Nurse Practitioner who can contribute as a team member, to work under supervision and to work independently. You must be skilled and have the capability to cope effectively with patients, families and others while maintaining confidentiality.
Specific qualifications for this Nurse Practitioner role include:
- A minimum of three (3) years of Home Care or Hospice nursing experience with at least a year of home health or hospice supervisory experience.
A degree or certificate in nursing from an accredited school of nursing and licensed to practice in the appropriate state.
- Certification as a Nurse Practitioner
- Prior experience in program development, building new teams, and end of life care is highly desirable
