We are looking for a Registered Nurse for the Hospice department at St Joseph Health!
Location: Memorial Hospice - Santa Rosa, CA
Schedule: Full time, 72 hours bi-weekly
Shift: 12 Hour, Night
Works a flexible schedule to help meet staffing needs for hospice patient/family care,
admissions, consultations and evaluations. Identifies patient/family problems and
needs and implements nursing interventions based on an IDT plan of care and
coordination of services with team members. Evaluates the results of interventions and
adjusts the plan accordingly. May provide coverage for Intake, Patient Care
Coordinator and weekend triage with appropriate training.
- Demonstrates proficiency in the assessment, evaluation and timely/accurate
documentation of the hospice patient.
- Demonstrates knowledge and skill in management of pain and symptom including
- Demonstrates proficiency in providing patient care services and coordinating
services with IDT members.
- Demonstrates knowledge of the regulatory requirement for providing care to
patients in all settings (e.g. home, SNF, Assisted Living) and the differences
between these settings.
- Demonstrates skill and proficiency in infusion therapies including use of
- Demonstrates skill and proficiency in assessing appropriate use of therapies for
parenteral and enteral nutrition.
- Demonstrates competency in respiratory therapies including use of associated

Education: Registered Nurse
Experience: Hospice, home health, or palliative care experience preferred.
License/Certification: California Registered Nurse, CPR Certification
