Complex Care Service NP/PA Lead

A full-time position as a Nurse Practitioner (NP) or Physician Assistant (PA) in the Division of Hospital Medicine, Department of Medicine at the University of Colorado School of Medicine, Anschutz Medical Campus, to join the Hospital Medicine Group (HMG).
The NP/PA will work in collaboration with the Department of Medicine/Hospitalist faculty and to provide clinical care to the following types of patients: medically complex patients with complex ongoing care and social needs. Additionally, the NP/PA may also provide care to patients in observation status, inpatients, floor and step-down level patients requiring medicine consultative services, and outpatients as needed, though less than 20% of the position.
Specific duties will include but not be limited to: Daily ongoing care of the medically and socially complex patients in partnership with care management and social work and other key teams, proactively planning for complex discharge needs, assessing and addressing acute medical issues as they arise, coordinating with patients, families, and care takers to ensure patient and family centered care, attend complex discharge meetings each week.
Additionally, you will be expected to spend time each month tending to non-clinical duties such as quality improvement efforts and administrative obligations. Administrative responsibilities will include input into hospital systems, development of critical pathways, committee work and projects aimed at improving the hospitalist group mission, and development of the complex care service. There will also be opportunities for teaching and scholarly work. You will also be expected to attend the group's meetings and educational events. Other duties may be assigned as need arises.
The NP/PA will be expected to mentor, coach and precept NP/PA fellows in the Division of Hospital Medicine Advanced Practice Fellowship Program.
