Full-time Program Nurse - Licensed Practical Nurse

Alzheimer’s Community Care seeks to hire two full-time Licensed Practical Nurses for our Lake Worth and Pahokee Specialized Alzheimer’s Adult Day Centers. This is a full-time position, no evening or weekend hours. Lake Worth is the 9:30am to 5:50pm Monday through Friday shift and Pahokee is the 8:30am to 4:30pm Monday through Friday shift. Recent LPN graduates will be considered.
The Program Nurse provides coordination and supervision of the basic health services and needs of the patients in the Specialized Alzheimer’s Day Center (SADC). The Program Nurse is responsible for the administration of patient medication, as directed by prescription label, physician orders, and accepted nursing practices. The Program Nurse supervises/assesses each patient’s health needs and also assists in the design/implementation of the activity schedule. Responsibilities include: the development of individual care plans; identification of recurrent patient problems, symptoms, and behavior changes in written progress notes; alterations of care plan to meet changing patient and family needs. The Program Nurse interacts with patient physicians, when needed. He or she assists with and supervises the help given for patient’s personal care needs including, but is not limited to, toileting, bathing, grooming, dressing, eating, and ambulating. In the absence of the Program Manager, the Program Nurse is designated as the Program Manager and is responsible for the supervision of the program staff & volunteers.
The successful candidate will possess excellent verbal and written communication skills; the ability to handle multiple tasks simultaneously and still maintain flexibility; excellent time management skills; proficient in all Microsoft Office suite products; and work independently and as part of a team. The successful candidate must be able to enhance the image of the organization in person, by telephone, and in written communications. Reliable transportation is a requirement.
A clear, active Florida LPN license is required, in addition to a valid driver’s license. Must pass AHCA level II criminal background screening and drug screening. A LPN with 2-5 years’ experience in a clinical setting (or psychiatric/crisis intervention background) preferred, although recent graduates will be considered.
