The Licensed Practical Nurse (LPN) will provide care and health services to patients by following the Plan of Care and instructions from the Registered Nurse (RN) Case Manager. Care, education, and treatments will be provided in the patient's residence which may be a home, facility or nursing home. The LPN must function with a large degree of independence and draw on their education and experience to provide care and treatment in a variety of conditions. A hospice LPN may also be scheduled to provide 1:1 continuous care for hospice patients who are actively dying and need skilled services. The LPN serves as part of the interdisciplinary team and will participate in team conferences to show observations, interactions, and barrier to meet goals.
Graduate of Vocational School, Technical School, or Nursing Program And 2+ years Experience working within the field
1+ years Medical or surgical experience
Current License as Licensed Practical Nurse in New Jersey
Current CPR Certification
Valid Automobile Insurance
Valid Driver’s License
