The Long TermCare (LTC) Social Worker Care Coordinator (SWCC) supports the provisionof care coordination in a manner that recognizes the Enrollee, medical hometeam, and (LTC) or supportive living facility (SLF) staff as essential partnersin the Enrollee’s care. The care coordination services are offered at theLTC/SLF facility.The LTC SWCC completesthe assessment of healthcare needs and/or the social determinants of healththat affect the provision of care. In collaboration with the Enrollee, the LTCSWCC uses assessment results to develop and implements a care plan to supportcare goals.Works with multipledisciplines to achieve the desired outcomes for the Enrollee.
·Licensed as a Social Worker in the Stateof the Illinois
·Two (2) years care coordination or workexperience in a collaborative multi-disciplinary model
·Two (2) years of healthcare workexperience
·One (1) year of experience in long-termcare environment, i.e. nursing home or rehabilitation facility
·ValidDriver’s license and mandatory vehicle insurance or other means oftransportation
Whenapplying for employment with the Cook County Health & Hospitals System,preference is given to honorably discharged Veterans who have served in theArmed Forces of the United States for not less than 6 months of continuousservice

A copy of a valid State ID Card or Driver’s Licensewhich identifies the holder of the ID as a Veteran, may also be attached to theapplication at time of filing.
