Contact patient as directed in compliance with Medicare and other payer program requirements
Consistently documents patient interactions and enters clinical data into EMR
Accurately utilizes Watson Clinic protocols, care coordination program guidelines, and patient-specific provider treatment plans to
a. educate Watson Clinic patients on managing conditions/diseases
b. promote patient self management
c. assist with coordinating health care services and
d. communicate with physician-lead care teams
Maintains excellent communication skills with patients to promote adherence to each patient's specific provider treatment plan.
Tracks activities in departmental spreadsheets and databases.
Required Education and Experience: Graduate of an accredited Licensed Practical Nurse program.Florida licensure required. Familiarity with clinical documentation in a practice group or hospital electronic health records (EHR). Experience with electronic health record (EHR) data abstraction to support quality project goals. Proficiency in excel required.
Preferred Education and Experience: 5 years in a hospital, clinic or physician’s office, case management experience, care coordination preferred. Prefer experience in both hospital and outpatient clinic setting.
Additional Eligibility Qualifications: With training/direction from the department leadership:
1. Ability and initiative to become proficient with quality measurement specifications related to group practices, e.g. CMS MACRA and MIPS measures, National Committee for Quality Assurance (NCQA) Health Effectiveness Data Information Set (HEDIS).
2.Ability and initiative to become proficient with accreditation standards that pertain to quality initiatives and care coordination activities, as outlined by the Accreditation Association for Ambulatory Health Care, Inc. (AAHC) and NCQA Patient Centered Medical Home standards.
3. Ability and initiative to become proficient with the various value based payer programs.
