What You Will Do
Under general supervision of the executive director and in collaboration with the administrative team, the medical director develops, implements and evaluates the medical practice policies and programs. The medical director shares in the administrative functions that directly impact medical services and collaborates with the management team in overall planning and budget activities and assures delivery of quality services to all residents/participants.
Responsibilities
- Supervises three nurse practitioners and one physician; responsible for oversight of all PACE medical care
- Facilitates strategic clinical relationships with physicians and leadership to develop a plan for growth of clinical practice
- Ensures that clinical programs are in compliance with governmental and regulatory requirements
- Implements utilization review, quality improvement systems, educational programs and offerings
- May provide medical services to patients in an assigned area
- Assessment
- Provide initial and ongoing participant assessments, care plan development and implementation of care plan
- Obtain a complete health history and records the findings in a systematic organized manner
- Perform physical examinations, orders and evaluates appropriate laboratory and diagnostic tests, and records in a systematic manner
- Identify medical, social, cultural and financial problems
- Perform functional status evaluation and/or mental status examinations using standardized procedures
- Identify and describe behavior patterns of the chronically ill
- Triage status
- Treatment
- Provide preventative health care and health promotion for ABCS PACE participants
- Treat and manage acute chronic illnesses in the day health center, nursing home, group home or in participants home
- Regulate and adjust medications as needed
- Facilitate medical specialist consultation as needed
- Educate clients and families regarding health maintenance and their chronic medical conditions
- Provide inpatient attending coverage responsibility when participant is hospitalized or admitted to a nursing home and communicates participant status to ABCS PACE interdisciplinary team
- Work as supervising physician with the nurse practitioner or clinical nurse specialist at that site
Evaluation
- Serve as a source of geriatric and medical knowledge for the interdisciplinary team
- Maintain an active role in the interdisciplinary team process
- Evaluate the effectiveness of the participants plan of care and revise as appropriate
- Participate in the development of the comprehensive plan of care and ongoing monitoring of the participants health status
- Maintain current, accurate documentation of health care services provided and coordinated or contracted
What You Will Need
Education and Experience
- M.D or D.O degree from an accredited medical school; fellowship in Geriatrics is preferred
- Board certified in one of the primary care specialties
- Must be licensed to practice medicine in the state where employed
- Strong geriatric clinical experience required
Certificates, Licenses, Registrations
- Valid drivers license
- Physician must provide current state medical license, DEA registration, BNDD and staff privileges at Ascension Living contracted hospitals
- Licensed CSR-Prescriptive Authority credentialed from current state Board of Pharmacy
- Licensed Physician MD/DO with Prescriptive Authority credentialed from the current Board of Medicine
