CitySquare’s Medical Respite Program is part of an external community partner collaboration working with the unstably housed and/or homeless population that have exited the healthcare system and medical respite care and do not have stable housing to return to. The CitySquare responsibility of the collaboration will consist of a case manager, housing stability specialist, community health worker (CHW), and a manager that places neighbors exiting the hospital and medical respite care into transitional housing with ongoing follow-up support.
The Case Manager will provide intensive case management to individuals placed into the CitySquare’s Medical Respite transitional housing program. This position requires knowledge and experience in housing first, harm reduction, mental health, addictive disorders, motivational interviewing, and benefits eligibility. The Case Manager will be responsible for helping with new move-ins, conducting intake and assessments, developing individualized short and long-term plans, conducting home and office visits, provide crisis management and intervention, provide support for neighbors, increase their social capital, assist with mainstream benefits and provide referrals to established agencies, charities, and government programs. Case Manager is responsible for helping participants get placed into transitional housing, provide supportive services and ongoing support to those enrolled in the program, and help them to move on to achieve successful permanent housing placements.
The Case Manager will work cross collaboratively with the medical respite team and collaborative partners and will be required to provide field-based case management traveling to neighbors’ homes once they are placed in housing.
Education: Bachelor’s degree in Social Work, or a related field, is required. Masters preferred.
Experience: One (1) year experience in case management, experience with highly vulnerable populations, homeless population preferred. Orientation to best practices in behavioral health services for vulnerable populations.
Licensure/Certification: Licensed from an accredited university in social work preferred.
Additional Requirements: Excellent communication and critical thinking skills. Positive attitude, flexibility in a highly dynamic environment and able to handle crisis situations under pressure. Self-motivated with ability to take initiative and be resourceful. Ability to prioritize and manage multiple projects/tasks. Must be proficient in MS Word, Excel, and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license, reliable transportation with valid insurance. Willing to transport program participants, as needed. Willing to work with homeless individuals with disabilities (physical, mental and/or substance abuse) who may or may not have criminal backgrounds. Willing to have an office in an apartment located onsite at our housing program locations at various scattered site apartments, if needed.
