Sunnyview Rehabilitation Hospital is in need of a Nurse Evaluator (NE) for the Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD) Home and Community Based Medicaid Waivers.
The selected candidate will be a Sunnyview Employee working for this NYS-DOH Grant Funded position. This employee will support the mission and functions of the Capital Region Regional Resource Development Center for the NHTD/TBI Waiver Program.
Specific duties include:
-Assisting the Lead Regional Resource Development Specialists & Assistant Regional Resource Development Specialists in assessing potential participants as to their medical/safety needs & appropriateness of referral and eligibility for Waiver services.
-Serve as back up during Lead RRDS' absence for service plan approval & Notice of Decision (NOD) authorization.
-Assist the RRDC staff & Provider Agencies by reviewing & administering eligibility assessments in timely manner & as appropriate including UAS-NY Assessments
-Review all medically based incident reports & sevice plans as needed and/or as requested by RRDC staff
-Provide trainings, technical assistance & education to waiver providers & RRDC staff on clinical aspects of the waiver program & share expertise with other NE's
-Act as a liaison between participants, families, service provider agencies, and all applicable community/government entities re: medical/safety needs & issues
- Valid NYS RN License
- 3+ years RN experience, preferabbly in the delivery of home and community based services.
