LifeLong Medical Care has a unique opportunity for an experienced part-time or full-time advanced practitioner to join our health centers in Pinole and Rodeo, CA, providing full-scope family medicine primary care to a diverse patient population. This position will work with other health center staff to provide primary care services and will have the opportunity to take part in the clinics' growth and development.
The ideal candidate must commit to providing high-quality outpatient primary care dedicated to building a collaborative team model of care, and ideally experienced working with the under-served. Bilingual in English and Spanish is highly preferred.

We provide medical, dental, behavioral health, and school-based services at 16 primary care sites across Oakland, Berkeley and west Contra Costa County. Primary care responsibilities include: diagnosis, treatment, consultation and follow-up care, medication orders, and interpreting results of clinical tests while collaborating with a supervising physician with the goal of delivering comprehensive care that improves patients' quality of life and prevents unnecessary re-admissions to acute settings; provides triage services; maintains accurate medical records through our electronic health records system, Epic.
- Current California Nurse Practitioner license
- Current and unrestricted DEA license
- Minimum experience: one year providing high-quality health care to a diverse patient population including working with the undeserved
- Strong interest in caring for a diverse, multi-cultural patient population in a community-based setting
- Ability to communicate and work effectively within an interdisciplinary team
- Ability to work independently with minimal supervision
- Community health center experience preferred
- Bilingual in English/Spanish highly desirable
