The Nursing Supportive Services Manager shall have the responsibility and authority to carry out the policies, procedures, and standards of the PACE Center(s) regarding the assessment, planning, organizing, and directing home care services for PACE Program participants and ensures adherence to and compliance with licensing and regulatory requirements. Responsibilities include training and oversight of planning, development, service, implementation, and coordination of related services and the overall leadership and administrative operation of home care services focusing on providing quality cost-effective care in the home setting. This individual shall be the liaison between AltaMed PACE and the Home Caregiver Services Vendor to ensure contract adherence, quality control and process improvements for the delivery of home care services as needed by participants. Shall be responsible for budget and fiscal effectiveness of the home care services through collaboration with PACE Center leaders, Nursing and Operations. This individual builds from the resources of the PACE program to plan and direct services to meet the needs of the individual and their family within the home setting. The Nursing Supportive Services Manager respects and promotes the mission, vision, and values of AltaMed PACE.
1. Education, Training and Competency Validation
a. Participate in interviewing, on boarding, and skills validation of LVN Home Care Coordinators
b. Evaluate HCC role and skill in IDT. Collaborate with the Interdisciplinary team to evaluate and re-evaluate caregiver hours by supporting the
HCC LVN. Support the HCC LVN while participating in Interdisciplinary Team (IDT) contributing to the participant plan of care. Communicates
any changes in participant.
c. Assesses, train and teach PACE site nursing staff: RN, LVN, Home Care Coordinator (HCC) LVNs using the nursing process, the home care
needs of a frail elderly population, to ensure development of age appropriate accurate assessments and development of plans of care.
d. Attends staff, scheduled IDT and Care Coordination meetings if needed.
e. Review, discuss and educate approved caregiver hours and assignment plan with HCC LVN Document in accordance with AltaMed PACE
departmental guidelines and within the established time frames.
f. Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring
continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary
communication and discharge planning.
g. Coaching, mentoring and Evidence Based Leadership development for PACE RN supervisors as needed. Implement training for RN Supervisors
in evaluating the day to day work of HCCs in executing participant’s plan of care
h. Provide and/or implement a mechanism for on the job training, continuing education, and in-service education of staff. Acts as a subject
matter expert in the development of performance, process, system and/or behavioral improvement plans.
i. Review nursing documentation, diagnosis and care plans, which establish goals and incorporate therapeutic, preventive, and rehabilitative

c. Analyze existing service providers and relationships for financial performance and develop appropriate strategies to improve quality,
satisfaction and Utilization.
d. Assessing, implementing and maintaining program policies and procedures to achieve participant home care service goals and requirements
and ensure that they are met.
e. Evaluate and develop program in accordance with needs of participants served.
f. Monitor, measure and providing analysis of Home care service delivery operational performance, (may be daily, weekly, etc.)
g. Designs and evaluates processes to improve systems and patient care results across the continuum of care.
h. Implement process improvement or action plans when appropriate and assuring compliance with program goals of participant satisfaction,
fiscal Responsibility and ensuring services which support participants remaining in their homes safely.
i. Serves a leadership role in regulatory audits for both preparation and as subject matter expert to ensure adherence to regulatory requirements.
j. Monitors, reviews and audit initial assessments of patient and family to determine appropriate level of service/ care.
k. Fiscal accountability, maintaining the business line resources and training needs.
l. Ensure that HCC LVN’s/CM RN are performing reassessments within 24 hours following a change in condition or discharge from an inpatient
setting, or a request in change in home care hours including care plan development.
m. Ensure that discharge planning needs is part of the care plan development.
3. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future
legislation; enforcing adherence to requirements; advising management on needed actions.
