JOB DESCRIPTION FOR OCCUPATIONAL THERAPIST -
As a member of the multidisciplinary team, the Occupational Therapist is responsible for the occupational therapy treatment of patients and performing related tasks in accordance with the physician's orders, the plan of care and the policies and procedures of the Home Health Agency.
1.Check patient's vital signs, record appropriately and report to Case Manager if necessary.
2.Perform initial patient evaluation and periodic re-evaluations using appropriate assessment tools for the patient's diagnosis and/or condition.
3.Establish an appropriate plan of care which addresses the patient's functional problems.
4.Identify appropriate short and long-term goals for each patient.
5.Perform appropriate therapeutic interventions to achieve the patient's maximum function and independence.
6.Instruct the patient and/or caregiver in appropriate home exercise programs.
7.Maintain clinical records on all patients including, but not limited to daily progress notes, which reflect patient status and progress towards short and long term goals, plan of cares, supplementary physician orders, re-evaluations, and discharge summaries.
8.Perform documentation according to the Agency's policies and procedures.
9.Maintain regular communication with the Home Health Therapy and Staffing Coordinators and other members of the interdisciplinary team regarding progress of the patient, changes in status, and discharge from services.
10.Maintain communication with the physician regarding status of the patient including, initiation of treatment plan, changes in status, and discharge from care.
12.Participate in patient care conferences as needed.
13.Attends appropriate in-service programs to keep current in field.
14.Performs other duties as assigned.
1.Current OT license issued by the state of employment.
2.Bachelor's degree, Master's Degree preferred from accredited school.
3.Successful passing of the National OT Examination.
4.Minimum of two (2) years experience in acute care or rehabilitation setting.
5.Current CPR certification.
6.Compassionate and interested in working with the elderly population.
7.Experience in office procedures:
a.Ability to work independently with minimal supervision.
b.Ability t resolve problems and set priorities.
c.Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail.
d.Pleasant telephone demeanor with exceptional customer service skills.
e.Basic typing and computer skills.
f.Ability to work cooperatively with others and multi-task in a busy office setting.
9.TB Test or Chest X-Ray 6 months prior to employment or within 30 days of employment.
