As part of the Community Health Team, the Panel Coordinator is an integrated Patient Centered Medical Home (PCMH) position responsible for clinical patient panel management activities.Under the general supervision of the practice Clinical Coordinator with matrix reporting responsibilities to the Community Health Team Lead, the Panel Coordinator works closely with associated primary care providers, practice staff and health team members to assist with patient self-management, patient panel management, data collection efforts, reporting and analysis.
This position develops/produces reports utilizing the electronic health record (EHR), Excel, Access or other relevant reporting tools. This position routinely shares quality performance reports with the practice clinicians, PCMH leadership, while teaching/leading the practices in completing patient panel management efforts.The Panel Coordinator will maintain absolute confidentiality of all referred outpatient records, medical treatment and diagnosis and abide with all CVMC and Medical Group Practices’ policies and procedures.
Prior knowledge of system management software applications (Excel, Access, EHR), process improvement (PDSA), and change management leadership required.Prior knowledge with the use of an electronic health record (eClinicalWorks) is preferred.The understanding and practical use of databases and to effectively communicate through the use of reports, graphs, and supporting narrative is a must.
Candidatemust have graduated from an approved Medical Assistant course or healthcare related education or its equivalent. Minimum of three years of medical office experience or equivalent is required.Strong computer proficiency including all Microsoft Office applications is absolutely necessary.The incumbent must possess strong and effective organizational skills; effective and tactful written, verbal, and nonverbal communication skills; the ability to recognize opportunities for improvement through patient, clinician or staff interactions; professionalism, sensitivity, empathy and respect; and must be a proactive/forward looking individual.
