Participate in activities and events in our local community, including health fairs, presentations at senior buildings, activities at churches, food pantries, and practically any place where there are a group of older adults.
Develop strong relationships with the people in their community
Coordinate activities in our community center including entertaining parties, bingos, trivia and karaoke. We also host regular educational events on specific health topics, benefit programs, ESL and creative writing classes. Finally, we meet with individuals one on one to address their specific concerns.
Encourage eligible prospects to attend a welcome visit with our doctor and care team to experience the Oak Street Health care model.
A large portion of the day is spent calling and following up with people we have met and who have not yet become patients.
Developing a network of referrals and service providers to continuously prospect new patients
Other duties as assigned

A high energy level and an orientation toward solving problems
Strong telephone skills for reaching and persuading patients and prospective patients to come into the center
Ability to create engaging programming, which would include: teaching classes, assisting older adults with computer classes, and other in-center activities
An orientation toward service--serving beverages and snacks and providing assistance with visitors to our community center with a great attitude
Ability to maintain a sparkling clean and well-organized community center
Someone who is driven by a performance-based compensation model where part of your total pay is determined upon meeting a quota and achieving goals
Availability to work some weekends, as, due to the flexible hours and nature of the position, Outreach Associates generally work at least two Saturdays or weekend events per month.
Computer proficiency- must be able to learn and adapt to various computer systems used in this role
Access to a vehicle that you can use every day to drive to events and activities in the community is required. Mileage is reimbursed
Familiarity with Customer Relations Management (CRM) systems is preferred
Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where required in the job title)
US work authorization
