Job Title: Patient Transfer CenterCoordinator
Roles amp; Responsibilities:
The Patient Transfer Coordinatorfacilitates the inter-hospital transfer of patients from outside affiliates aswell as other hospitals into the Mount Sinai Health System. The PatientTransfer Coordinator is responsible for ensuring that the patient meets all MountSinai’s clinical, financial and regulatory criteria for transfer.
Duties and Responsibilities:
Serves as the point of contact for referrals andobtains necessary information to initiate the patient’s transfer into orbetween member hospitals of the Mount Sinai Health System.
Receives transfer requests fromphysicians, affiliate represent or families. Information includes demographic,insurance and clinical data.
Responsible for the complete and accurateregistration of all patients in an expeditious and positive manner.
Coordinates and secures inpatient referrals toappropriate medical services as needed for dedicated patient population.
Connects with accepting Mount Sinai Physician todetermine clinical appropriateness for the transfer.
Performs follow up communication to patientfinancial services, insurance companies, physicians, case managers, socialworkers, nurses, staff, family members, and others regarding expeditingapproval of a patient transfer.
Collaborates with Admitting and Throughput (BedManagement) to determine availability of appropriate bed.
Arranges for appropriate ambulance to bedispatched based on patient acuity (e.g. BLS, ALS, and Critical Care Medic).
Communicates with the referral and acceptinghospital of approval and time to transfer and admit patients.
Identifies, analyzes, organizes, and solvesproblems and issues in a timely, effective manner and collaborates with othersto make sound, timely decisions.
Maintains and provides daily, weekly and monthlystatistics for submission to Administration.
Advocates for patients, families, and referringphysicians to promote overall satisfaction with accessing and utilizing MountSinai Health System.
Monitors patient progress and communicatesinformation between patient, referrer, family, insurance company and treatmentteam.
Promptly answers and screens inquiries withstrict adherence to confidentially agreements, policies and procedures.
Documents all patient encounters and enters allclinical data into referral tracking system.
Demonstrates knowledge of hospital, departmentaland care center standards, policies, procedures and guidelines as well asregulatory agency standards.
Assists with new and current initiatives (e.g.Community Paramedicine)
Other duties as assigned by Manager or Director.
HS diplomarequired as well as NYS paramedic certification, BCLS, ACLS and PALS.
Minimum of 3 years comparable and relatedexperience.
Must be able to triage clinical needs andcoordinate all aspects of clinical care for patients.Customer service and interpersonalcommunication skills is a priority.Computerliteracy including Microsoft Office, Power Point, EXCEL, Web-based database.
