ThePharmacy Departmentis looking for aPer-Diem Pharmacist.
The staff pharmacist is responsible for assuring the effective and efficient provision of pharmaceutical care services. He/she must perform all duties necessary to provide for effective daily operation of departmental services and programs. The pharmacist is proactively involved in safe, rational and cost-effective therapeutic decision making. In the course of performing his/her duties, the pharmacist is responsible for the activities of pharmacy residents, students and supportive personnel as assigned in all practice areas. The pharmacist reports directly to the immediate supervisor of the assigned practice area.
Position Qualifications Include:
Must have a minimum of a Bachelor of Science degree in pharmacy and/or Pharm.D. and be licensed or eligible for licensure in Illinois.
The incumbent must possess effective verbal, interpersonal and written communication skills.
Collects/obtains and evaluates physicians= orders for proper disposition of the order, as well as therapeutic appropriateness, proper dosing, drug interactions, allergic reactions, route and rate of administration and therapeutic redundance.
Clarifies physicians= orders when necessary through direct and timely contact with prescriber or designated practitioner.
Enters physicians= orders or supervises technician order entry into the patient record in the pharmacy computer system. Documents allergies, diagnoses, weights and ages where appropriate.
Performs daily review activities, (e.g., minibag audit, drug interaction monitoring, diagnosis and allergy review).
Provides general supervision for technical support in their assigned operational area.
Monitors and coordinates workflow with established delivery schedules.
Attends CPR emergencies; provides coverage for other pharmacists involved in these emergencies.
Provides general and/or patient specific drug information as required to other health professionals.
Conducts monthly nursing unit inspections of mediation storage areas as assigned.
Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics Committee.
Participates in ADR reporting program.
Participates in drug use evaluation (DUE) as assigned.
Actively participates in departmental clinical skills development programs.
Attends medial center and/or departmental in-service or continuing education programs.
Assists in the training of new employees, support personnel, residents, and students.
Assists in maintaining required quality assurance records.
Responds to the department=s Disaster Plan as prescribed by medical center policy.
Reviews policies and procedures when requested.
Belongs to local/state/national professional pharmacy organizations; attends meeting.
