Under the direction of the Medical Director, PPS and the Practice Manager, the Perinatal Case Manager is expected to develop and implement a Case Management program within the Obstetrical Practices to identify women at risk, assess their resources and challenges, and optimize their psycho social, medical and financial health during the prenatal and post partum period.
Patient Population Served:
The Perinatal Case Manager is expected to demonstrate the knowledge and skills necessary to provide care based on the physical ability, intellectual development, psycho social, education and safety needs of the pregnant and newly delivered women in our obstetrical practices.
Essential Position Functions:
Develop, in concert with providers, nursing, other social work staff, and the practice manager, a Perinatal Case Management program utilizing comprehensive, holistic, and evidence based tools and practices.
Facilitation and coordination of care and services: Communicates/collaborates with all members of the health care team, including the patients regarding patient's needs, plan and response. Considers patient needs to include medical, physical, psycho social, spiritual, financial and emotional aspects.
Completes psycho social assessments utilizing clinical protocols.
Participates in evaluation of clinical data and formulation of treatment plans and recommendations.
Documents in clinical chart according to hospital policy.
Develops and maintains professional relationship with providers, clinical support staff, non-clinical support staff, hospital departments, patients and their families.
Supports Departments' customer service efforts. Provides service excellence to patients and their families, medical staff, peers, support services and other hospital personnel. Provides immediate and satisfactory problem resolution for valid customer complaints or refers complex customer service problems to the Practice Manager.
Participates in the department's quality improvement initiatives by collaborating in developing and implementing effective quality initiatives.
Demonstrates knowledge of, and support for, hospital Mission and Values Statement, department goals and objectives. Assists in the formation and accomplishment of department goals and objectives.
Assumes responsibility for own professional development and ongoing education. Attends required hours of continuing education per year.
Liaison for community services referral sources.
Provide appropriate referrals for each family assisting with access to community resources and collaborating with community agencies as appropriate.
Provide information, advocacy support and access to resources for breastfeeding, family planning and interconceptional care.
Connect families to a wide range of community resources and services responsive to the needs of families with pregnant women and/or young children.
Interface with our pediatric practice, Providers and Social Worker regarding optimizing continuity of care for at risk families.
Non-Essential Position Functions:
Hospital and community education. Performs other duties as needed or as judgement dictates.
Bachelor's in Social Work. Experience in Maternal Child Health preferred. Must be resourceful, creative, organized, flexible, self-motivated, and willing to participate in the creation of a first class program. Requires excellent communications skills, the ability to be patient, open-minded, non-judgmental, friendly, and able to work with diverse populations.
