Under the supervision of the center manager, the personal care attendant/CNA is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. Provides transportation for the participants from their homes to the center and for appointments during the day, and for field trips as part of the activities program. Delivers in-home meals and provides delivery and pick-up of supplies and medications for participants. Cleans vehicles, checks regularly for safety, and maintains scheduled maintenance.
1. Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring. Report on personal care services provided and the self-care abilities of participant, including changes noted in participant’s condition, as well as any issues noted when transporting the participants.
2. Drive vehicles in a safe and responsible manner utilizing defensive driving skills following assigned routes.
3. As part of the health care team, provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming. Assist in the maintenance of physical activity, exercise, and walking; including loading and unloading participants in and out of vehicles, assisting participants into and out of their homes. Transport participants to all requested appointments that have been arranged by the transportation coordinator or dispatcher.
4. Coordinate scheduling of participant rides with the transportation coordinator or dispatcher.
5. Provide nursing care as assigned in a safe manner, deliver in-home meals, supplies, and medications to participants.
6. Responsible for participant safety, identify safety hazards and emergency situations, and initiate corrective action as able; maintain safety and cleanliness of assigned vehicle and coordinate scheduling of van maintenance with transportation coordinator.
7. Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance.
8. Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping; assist participants in their home environment with simple tasks when necessary, such as put on socks, shoes, jackets, etc.
9. Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.
10. Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participant’s expressed wishes. Prepare and submit timely written reports as required.
11. Participate in continuing education classes and maintain professional affiliations and any required certifications.
12. Attend and participate in staff meetings, participant care planning, in-services, projects, and committees as assigned.
13. Be flexible in schedule of hours worked. May also require use of personal vehicle.

• Ability to read, write, and speak English.
• Knowledge of the principles and techniques involved in relating to frail elderly persons
• Effective oral and written skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants
• Demonstrates sensitivity to the needs of older adults as evidenced by a caring approach, patience in providing care, and responsiveness to participant requests
• Ability to follow directions, verbal and written instructions, and adapt work schedule under supervision
• Ability to follow through with instructions and directions provided by supervisor, RN, QAPI coordinator, primary care physician, and/or medical director
• Ability to utilize appropriate safety measures when providing care
• Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner
• Ability to work effectively and harmoniously as part of an interdisciplinary team, the elderly, providers of services, and peers in particular
• Function independently, have flexibility, and personal integrity.

• Computer skills required
• High School Diploma or GED
• Licensed with the Wyoming Board of Nursing as a CNA (see Additioanl Information for alternative licensure accepted)
• Minimum of two (2) years of documented experience working with a frail or elderly population
• AHA BLS
• Valid USA driver’s license
• Must have clean driving record that follows Cheyenne Regional Medical Center’s driving guidelines
• Experienced in transporting individuals with special mobility needs
• Experience in a long-term care facility or with a community-based geriatric program
