The ARNP/PA-C will be responsible for independently managing a cohort of heart failure patients in the outpatient cardiology clinic. These high acuity patients require ongoing comprehensive medical management. You will work collaboratively with another Health Care Specialist, Cardiology staff nurses, pharmacists, nutritionists, social workers, faculty Cardiologists and other cardiology specialists to enhance the quality of care provided to a high risk population. In addition to outpatient responsibilities, you will serve as a heart failure expert within the institution, bridging clinical responsibilities between Hospital and Clinic. You will play a key role in reducing heart failure admissions/re-admissions, LOS, and in improving the overall health of the heart failure patient population at this very busy level-one cardiac, stroke, vascular and trauma center. RESPONSIBLE TO: Administratively to the clinic practice manager, medically to the Cardiology section head or designee. Major Responsibilities: 1. Maintain the care of a panel of patients in Cardiology Clinic. 2. Obtain and record medical histories, perform physical examinations, order and interpret diagnostic tests, establish diagnosis and follow current standards of practice for prescribing medications and/or establishing treatment protocols. Recognize own limitations and consult when appropriate. Document findings, reports and plans of care, including patient education and appropriate follow up, in EpicCare. 3. Team liaison for Cardiology Heart Failure Clinic and Transitions from acute hospital care. 4. Attend and participate as a member of the Heart Failure QI (STAAR) committee (Quarterly). 5. Committee chair: Cardiology-Continuity of Care Nurses (CCNs) Transitions of Care work group 6. Attend Division faculty meetings 7. Attend Cardiology Grand Rounds 8. Clinic liaison for CCNs 9. Medical consult to CCNs and non-cardiology inpatient teams to help with timing of heart failure outpatient follow up at time of hospital discharge. 10. Support Cardiology Clinic RN visits for cardiology patients 11. Provide education to inpatient nursing as needed and as pertains to cardiology/heart failure 12. Precept ARNP/PA students 13. Process improvement projects: a. Transitions of care: STAAR and CCN-Cardiology work group b. Developing guidelines for CIED management c. Develop and maintain HF dashboard for population management d. Participate with enterprise efforts to enhance current EHR with respect to cardiology 14. Round with the in-patient cardiology team as available 15. Participate in HF team meetings, to discuss process problems and patient care scenarios 16.Participate in clinic and interdepartmental efforts and initiatives to facilitate a safe environment for staff and patient care, performing within the scope of licensure and in accordance with clinic, hospital and university policies/standards. 17. Monthly rounding meeting with cardiology clinic manager 18. Quarterly meetings with HCS director 19. Develop and maintain relationship with clinic staff, clinic manager, clinic medical director, interdisciplinary team and other hospital staff to ensure maximizing positive department and employee relationships. BLS, ACLS Licensure as either an Advanced Registered Nurse Practitioner in the State of Washington or a Physician's Assistant (PA) in the State of Washington, or equivalent legal license/certification; must be eligible to receive hospital staff appointments for adjunct staff membership by the organized medical staff of the hospital under Medical Staff By-Laws for Hospital-based positions.
