Shifts: Sunday-Saturday; days and evenings as needed
Originates schedules and supervises activity programming for the Swing Bed Program within the limits set by the attending physician(s) and with active support of Administration and Nursing Service. Completes activity assessment and develops the activity plan in accordance with the comprehensive assessment, the interest and the physical, mental and psychosocial well being of each patient/resident.
A. Develops an activity program whereby patients/residents have the opportunity to function at their highest physical, social, cognitive and/or emotional level.
B. Plans monthly activity programs in advance, develops activity monthly calendar to be made available to patients/residents and staff.
C. The Activity Program shall include games, mental quizzes or puzzles, music, divisional and restorative activities, etc., geared to individual patient/resident interests.
D. Maintains a library of activity program resources and materials.
E. Responsible for assessing, identifying, and developing patient/resident activity needs/plans.
F. Reviews and maintains proper documentation in the patient/resident medical record.
G. Provides consultation to Activity Assistants and patients on a regular basis for a minimum of two times per week according to activity needs.
H. Participates a member of the multidisciplinary care conference team attending conferences a minimum of once per week.
I. Demonstrates and supports through practice the protection of all patients/residents' rights.
J. Supervises activities provided for the Swing Bed Program patients/residents.
1) Assesses need for new equipment and requests as appropriate.
2) Serves as department liaison to other departments and physicians.
3) Assists with evaluating and training of activity assistants to assure the competency and skills are maintained.
K. Supports and abides by all PHO departmental and safety policies and procedures.
L. Performs other responsibilities as requested by the Director of Acute Care Services and/or Administrative Team.
M. Provides competent patient care to the above-indicated patient populations.
N. Acts in a professional manner reflecting the mission, philosophy and values of Mitchell County Regional Health Center and Mercy Health Network.
O. Maintains on going continuing education credits to maintain licensure/certification as required by the state and per HCFA Standards.
P. Demonstrates Guiding Behavior Expectations in all interactions with all parties.
Q. Adapts positively to the ever changing health care environment by supporting technology and process changes.
R. Documents patient information in the electronic/paper record in an accurate, timely manner.
S. Maintains active log-ons/passwords for computer software systems that support patient care and education.

A. Attends departmental/hospital in-services and meetings as required or requested.
B. Assists in CQI activities and process improvements.
1 High school education or equivalent.
2 Certified Activity Coordinator or other equivalent as per HCFA Standards.
1. Previous clinical experience in swing bed, SNF, with knowledge of long term care regulations preferred.
2. Previous healthcare experience desirable/preferred.
C. Special Skills & Aptitudes:
1. Desire to work with the ill, aged, physically and mentally impaired.
2. Understanding of the aging process.
3. Patience in implementation and working with patients/residents.
4. Ability to work as part of multidisciplinary health care team.
D. Must treat all employees, patients, residents and visitors non-judgmentally.
E. Must be able to communicate effectively in English, both verbally and in writing.
F. Must have basic computer skills.
ESSENTIAL PHYSICAL/COGNITIVE REQUIREMENTS
A. Must be able to lift periodically up to 10 lbs, and push up to 30-60 lbs occasionally.
B. Must be able to very frequently or continuous walk or stand.
C. Be able to see, hear, and distinguish sounds, distinguish temperature.
D. Must be able to communicate effectively in English, both verbally and in writing.
E. Perform detailed tasks periodically.
