The Center for Community Alternatives (CCA) is a leader in the field of community- based alternatives to incarceration. CCA promotes re-integrative justice and a reduced reliance on incarceration through advocacy, services and public policy development in pursuit of civil and human rights.
Recovery Care Manager, Syracuse RCOC: Support individuals who are in recovery and who may have current/prior criminal justice experience to participate in the Syracuse Recovery Network. Utilize Recovery Coaching/Peer Mentoring skills to demystify treatment and help participants access treatment and negotiate insurance barriers. Requires occasional evening and weekend hours.
Duties and Responsibilities:
Screen individuals interested in participating in the Syracuse Recovery Network to ensure they meet eligibility requirements.
Provide referrals to community-based behavioral/physical health services and follow-up on status of referrals.
Provide individual and group Peer Recovery Support Services, including: screening participants utilizing CCA’s Intake Form, Locator Form, and the OASAS Perception of Care Survey; providing ongoing support through Recovery Coaching/Peer Mentoring; assisting participants with family health and wellness and family reunification; facilitating and following-up on referrals to community-based service provider agencies.
Monitor participants’ recovery status and ensure that recovery support services provided are helping to sustain their recovery; conduct home and community visits, and transport participants to and from appointments, as needed.
Maintain participant records, collect and enter program required data into CCA’s database (at intake and at six-month follow-up benchmark), and generate reports, as requested.
Recruit, train and support Syracuse Recovery Network members to work on community outreach projects.
Facilitate peer leadership training curriculum. Supervise peers in the delivery of services.
Assist with the coordination of affiliation activities and recreational events.
With Syracuse Recovery Network members, testify at public hearings and community forums on issues related to the goals and objectives of the Syracuse Recovery Network.
Serve as a liaison with community groups interested in recovery issues.
Attend community stakeholder meetings on behalf of the Syracuse Recovery Network, as directed by Project Director.
Attend staff meetings as directed by Project Director.
Attend training programs as assigned.
Complete required paperwork in a timely manner.
Perform other duties as assigned by Project Director, Director of Recovery Services, Deputy Director or Executive Director.
Qualified Health Professional: CASAC, LMSW, LCSW, CRC, or AAMFT required. Must demonstrate knowledge of the community resources, existing network of behavioral health providers, and confidentiality regulations. Commitment to and experience in working with adults and youth with diverse cultural backgrounds who are involved, or at risk of involvement, with the criminal/juvenile justice system, HIV/AIDS, or substance abuse. Personal experience with recovery and/or criminal justice issues strongly preferred. Strong written and oral communication, project management and supervisory skills. Computer literacy (word processing, e-mail, internet research, data entry) required. Bilingual (English/Spanish) preferred. Possess a valid NYS driver’s license, a reliable vehicle able to transport participants, and appropriate level of auto insurance, as required by CCA.
