The RN Case Manager (RNCM) is a total quality client/family advocate that coordinates all care in a multidisciplinary method. A registered nurse who is responsible for the coordination of care of the child and family assessing needs and coordination of appropriate clinical options. The nurse case manager facilitates referrals to primary providers, specialists, healthcare facilities and community resources. Responsible for direct client care including a holistic approach to the care of children and their families. This includes educational, psychosocial, behavioral, physical, mental and emotional care/needs of the child. The nurse will also educate the client and family regarding immediate care needs and preventative behaviors.
RESPONSIBILITIES AND DUTIES
1. Provides exceptional customer service and quality care:
a. Seeks to provide professional and courteous service to the children and their families.
b. Participates in a team effort to fulfill area goals and work requirements. c. Maintains confidentiality in all situations and interactions.
d. Works as a team player and works effectively with minimal supervision. e. Follows chain of command with questions and concerns.
2. Conducts sound nursing assessments and utilizes appropriate established guidelines.
a. Demonstrates the ability to utilize and apply nursing process in a child care setting. Demonstrates the ability
to provide individualized care at all times.
b. Utilizes the appropriate guidelines and information based on the age of the child to foster independence and advancement.
c. Demonstrates good assessment, evaluation and observation skills. d. Performs multiple tasks prioritizing these tasks appropriately.
e. Flexible in changes that occur throughout the day including staffing, children, employees and nursing care. f. Handles emergent situations while remaining calm.
g. Reports situations immediately to the Site DON or designee.
3. Documents appropriate information in a timely manner.
a. Performs chart documentation at the time care/direction is given.
b. Utilizes proper procedure for all care/direction.
c. Documents or delegates this documentation for accurate and complete demographic assessment information on all children.
d. Uses dates, time and signature title appropriately on all documentation.
4. Maintains the following nursing responsibilities:
a. Coordination of all care to the children as directed by the physician according to the plan of treatment and care plan.
b. Reports condition of the child to all team members, supervisors, physicians and parents. Makes daily rounds or more frequently if indicated to gather this information.
c. Supervision of documentation making sure that it is accurate and appropriately done in a timely manner.
d. Assesses, evaluates, observes and manages systems established in the plan of treatment. Comprehends
procedures that require substantial specialized knowledge.
e. Ensures all judgments are made based on the holistic aspects of care of the child.
f. Performs direct client care including personal care activities, use of adaptive equipment, follow through of
therapy, administration of medications, treatments and procedures.
g. Maintains a clean, healthful and safe environment.
h. Fosters the family and client to achieve independence.
i. Seeks advice and assistance if needed.
5. Maintains professional responsibilities to stay up to date on standards regarding child care, PPEC regulations and The
Kidz Club policies.
a. Keeps current in The Kidz Club affairs via staff meetings and designated communications.
b. Records work accurately and appropriately.
a. Attends in-service education and required training including The Kidz Club in-services and additional inservices
to learn new procedures, applications and updates.
6. Assists in cleaning and maintenance of the facility.
7. Performs other duties as assigned by the Charge Nurse or Site Director of Nursing.
8. Follows all policy and procedures implemented by The Kidz Club as applicable to the job or duties assigned and in
relation to the employee handbook.
9. Maintains a current certification in basic life support.
10. Maintains a current valid RN Kentucky License.
Graduate of an accredited school of nursing with current RN licensure in the state of Kentucky.
BSN preferred. Basic personal computer skills and comfort with Windows Operating System. A minimum of six months current experience as a practicing RN with pediatric experience preferred. Current negative Tuberculosis skin test (PPD) required. Current CPR (Basic Life Support) required.
