We serve individuals 16 and older who are experiencing serve and persistent mental illness, mental illness concerns, and chronic homelessness. On any given day, Guild staff serves nearly 900 individuals in the Twin Cities metro area. Services are mobile, provided in the community when and where needed. By looking at the holistic wellness of each client we serve, we are able to facilitate healing, growth, and new quality of life. Purpose: This position is responsible for providing nursing services for individuals in residential and crisis services lines. Assists with the admission process including assessments and determines immediate recipient needs. his position provides a full range of community health nursing, health management, and community health services to individuals struggling with a serious and persistent mental illness. The position will work Monday to Friday evening shifts and provides on-call nursing as needed. Key Responsibilities: Required Skills, Knowledge and Abilities Knowledge of: Major mental illnesses and the needs of people with mental illness Psychotropic medications Health and community resources. Skills in: Human relations including understanding of and respect for boundaries, values, and personal differences. Organization and time management Creative problem-solving Oral and written communication with consumers, public officials, physicians and other medical professionals, other community agencies and all Guild Incorporated Staff. Assessing a wide range of recipient needs. Ability to: Establish rapport and trust with recipients, demonstrate confidence in and promote the self-esteem of others, and demonstrate genuine compassion and respect for people. Work independently and recognize when supervision is needed. Document information clearly. Recognize potential problems and crisis situations: ability to intervene and implement a solution. Teach individually and in a group setting in areas of Health and Medication Management. Be flexible and adapt to changing conditions in the workplace. Establish and maintain positive working relationships with community services providers and families. Provide on-call nursing services, which includes responding to all calls within 15 minutes, complete crisis wellness assessments (as needed) within 24 hours of admission. Complete wellness assessments in the IRTS Program within 3 days of admission. Use medical equipment such as blood pressure cuffs, scales, stethoscopes, accuchecks, otoscopes, wheel chairs, glucometers, needles, and syringes. Read mantoux tests and document. Assess medical issues and implement care needed, including a 911 call. Provide on-call nursing services on a rotating schedule, which includes o Responding to all calls within 15 minutes, o Completing crisis wellness assessments (as needed) within 24 hours of admission. o Completing wellness assessments in the IRTS Program within 3 days of admission.
