Provides education regarding diabetes self-care to inpatients, outpatients, community members, and their families/caregivers.Reviews patient information, obtains history, and assesses patient s current knowledge, barriers to learning, and learning preferences.Provides education as appropriate on diabetes pathophysiology, glucose monitoring, insulin self-administration, other diabetes medications, and other self-care topics as appropriate.Collaborates with primary diabetes educator and other healthcare team members including physicians.Provides education to inpatients in the hospital setting, and to outpatients and community members via individual appointments and/or group classes.Assists primary diabetes educator with data collection and earning/maintaining facility ADA/TJC certifications/recognitions.
Hours: each Friday, Saturday, and Monday from approx. 9:00am to 5:30pm; 24 hours per week.May need to be occasionally available other days and times to relieve primary educator.
-Current CA Licensure as RN.
-Bachelor of Science Degree in Nursing preferred.
-Minimum 2 years experience as an RN.
-Experience in care of patients with diabetes required; patient teaching experience strongly preferred.Candidate with strong knowledge of diabetes and diabetes self-care strongly preferred.
-Possesses basic computer skills.
-Clear, professional, personable communication.
-Bilingual English/Spanish preferred.
-Specialty Certification: Certified Diabetes Educator (CDE) preferred.Must obtain in a reasonable timeframe after hire if applicant not currently a CDE.
