The Intake RN is responsible for the screening of incoming calls, obtaining any relevant nursing and medical information necessary to interpret and facilitate the needs of the caller, referral coordination, ensuring physician orders for services are in place, physician maintenance, ICD-9 coding, processing referral information and for guiding non-professional staff through referral issues for Home Health, HME, Hospice and IV Pharmacy referrals.

SPECIALIZED KNOWLEDGE Associate's degree (A.A./A.S.) or equivalent from two-year college or an accredited school of nursing. Licensed in the state of Ohio. Minimum one year acute home health, hospice, IV pharmacy or HME experience.
