The MetroHealth System in Cleveland, Ohio is launching a search for a Senior Dedicated Care Physician.
The MetroHealth Medical Center is an outstanding academic medical center and has been a major affiliate of Case Western Reserve University School of Medicine since 1914. The primary goal of the Senior Dedicated Care Program is to provide holistic, preventive, wellness care, as well as chronic disease management, for patients who choose to enroll. This program will provide enhanced support for social determinants of health, additional touchpoints and time for patient care using telehealth and other tools, with a dedicated team of doctors. The goal is to provide a customized and personalized healthcare experience for Seniors who select to enroll in this program, achieve the tenets of the Quadruple Aim, and do so with the security provided by a larger academic health system.
The Senior Dedicated Care Physician must be Board Certified in Internal Medicine, Family Medicine, Med-Peds, or Geriatrics. A minimum of 2 years clinical ambulatory experience is required.
