Social Services Coordinator Job Summary:
Providesresidents and families with the support needed to transition to the facilityand the aging process.
Also, providescare and case management or interventions designed to promote person centeredcare.
Completes and documents initial resident assessments for all admissions and readmissions and all required ongoing assessments.
Assists new resident and family with orientation to community.
Advocates for the resident and family during the resident's entire stay with Bethesda including admission and discharge.
Researches and provides information on services to family members in need of additional support and resources.
Participates in Care Plan Conferences.
Coordinates problem-resolution process across departments that address resident, family or staff concerns.
Discusses current Medicare residents' plan of care and plan for discharge on a regular basis. Plansdischarges for residents by working with the resident and family to determine best discharge options.
Makesappropriate referrals to community resources.
Qualified candidates will also possess:
Bachelor's degree or related work experience
Excellent customer service and communication skills
Excellent organizational and follow up skills
Proficient with Word, Excel and Outlook Experience with electronic medical records a plus
