Social Worker will be Lic. MSW in Texas , with at least 3+ years of experience. Social Worker willwill be responsible for:
Performing discharge assessments on the floor and in the Emergency Department
Identifying High-Risk patients and assessing their potential needs
Working with a multi-disciplinary team withinhospital and outside ofhospital to secure services and referrals for patientsFollowing up with transitions patients who may readmit to thehospital and completing a discharge assessment, including a readmission assessment
Communicating daily via email, phone, or in person with the Transitions of Care NP and follow up with patients who may be in-house or in the emergency department
In addition to seeing transitions patients, this Community Connections Social Worker (CC-SW) will see patients in the Emergency Department daily that have been identified as in need of service. This can be a referral from the physician, nurse, UR nurse, or the patient and/or patients family or the CC-SW own review of records.
