The West Farms Residence services individuals who have been formerly homeless with special needs, while also offering lottery apartments to the community.
The Social Worker/ Case Manager will develop supportive relationships with clients; link them to community based medical and mental health care, as well as provide crisis intervention as needed. Fieldwork may be required.
Conducting and completing psychiatric and comprehensive assessments, service plans, progress notes, monitoring and re-assessing clients needs for treatment and providing community education on chemical dependency related issues.
Assessing clients to identify interrelated issues of substance abuse, health, behavioral health, and other care needs and determine appropriate internal and external referrals and preparing psycho-socials;
Providing individual and group counseling;
Facilitating psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks;
Assisting clients in achieving and maintaining functional improvements in self-care, employment, or personal relationships;
We are looking for a compassionate, flexible, energetic, patient and organized individual with extensive knowledge of community mental health resources, counseling processes and practices.
REQUIRED EDUCATION and EXPERIENCE:
Masters Degree in Social Work (MSW) required; New York State Licensed Masters in Social Work preferred;
Two (2) years work experience
Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) is a plus
Bilingual skills in English/Spanish a plus
