Aspirus is seeking a Social Worker – MSW to assist patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. Will work with patients and families to obtain psychosocial history, assists patients in resolving environmental difficulties interfering with attainment of maximum benefits from medical care, and conducts routine discharge planning. The Social Worker is a core interdisciplinary team member who, as a professional Social Worker, provides social work services to patients/families under the direction of physician.
Responsibilities will include:
Coordinates the provision of psychosocial services for patients/families.
Regularly assesses/reassesses the patients and family's social service needs and coping capacity related to illness.
Links patient and family with needed community resources to meet ongoing social, emotional and economic needs.
Develops relationships with community service organizations, long-term care facilities and government agencies.
Participates in interdepartmental programs and activities, community activities, and education of employees as assigned.
Ensures interdisciplinary team's data is incorporated into psychosocial plan of care.
Assures continuity in psychosocial care and services based on the plan of care.
Develops and refines program policies for social services to meet state and federal regulations and JCAHO standards.
Impacts organizations decisions related to social work care/services through participation on committees, conducting education offerings, and working on projects.
Possesses Wisconsin Certified Social Worker licensure or provisional license required.
Knowledge of social work theories, therapies, and techniques of casework processes, and principles of public welfare at a level normally acquired through completion of a Master's Degree in Social Worker from a social work curriculum accredited school or approved by the council on Social Work Education.
One year medical social work experience necessary to gain full knowledge of federal, state and local regulations related to discharge planning and knowledge of health and welfare resources and facilities.
Possesses and maintains a valid Wisconsin Drives license required.
Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability.
