- Examining and treating episodic acute illness and coordinating all medical services and consultations in a variety of settings, including the adult day health center clinic, acute care hospital, nursing home and participant’s home.
- Providing a minimum of monthly on-site follow-up visits to participants in nursing homes.
- Overseeing the care of hospitalized participants, including, when necessary, conducting daily rounds at hospital or overseeing Hospitalists’ or specialists care.
- Attending daily team meetings and communicating information regarding participant’s health status to the multi-disciplinary team.
- Providing quarterly physical examination of all participants and participating in regular meetings of multi-disciplinary team to discuss quarterly assessments, review care plan and revise if necessary.
- Provide on-going health education to participants as required.
- Completing documentation as required, including recording medical changes of participants and other necessary information in medical records.
- Providing 24-hour on-call coverage for medical care on a rotational basis with on-call physician group.
- Reviewing prospective participants’ medical records.
- Assessing prospective participants health status including obtaining complete history and performing thorough physical examination.
- Participating in Intake and Assessment team meetings to review multi-disciplinary team assessments and determine appropriateness of enrollment and, if accepted, develop treatment/plan of care for all new participants in coordination with other members of the multi-disciplinary team.
- Provides in-service training to clinic and other SSC staff as needed.
- Requests and authorizes consultations for participants care from SSC’s panel of contract medical and surgical consultants/specialists whenever such consultation is needed.
- Collaborates as needed with routine on-site specialists including the contract dentist, podiatrist, audiologist, optometrist, ophthalmologist and speech therapist.
- Coordinates all services provided to SSC participants in acute or nursing home settings, including overseeing care provided by specialty consultants and/or Hospitalists and working with the multidisciplinary team to monitor care.
- Participates in Utilization Review (UR) Committee and other UR activities and follows policies and procedures for utilization management.
V. Participates in quality assessment and improvement program (QAIP):
- Reviews and recommends policies and procedures for continuous improvement of medical/clinical care.
- Serves on Internal Quality Assurance, Pharmacy and Therapeutics and Infection Control Committees.
- Maintains membership in good standing and is credentialed to practice at Sutter Medical Center, Sacramento (SMCS).
- Assists Medical Director and Administrator with implementation of SSC’s Quality Assessment and Improvement Program (QAIP) and Utilization Management Plan.
- Contributes to efforts to enhance participant and caregiver satisfaction.
- Advises Medical Director and Administrator regarding development of clinical education for employees on issues related to aging and care of the aging and serves as educator as appropriate.
- Implements and adheres to all policies and procedures of SSC and Sutter Health .
- Attends supervisor’s and other administrative meetings upon request.
MINIMUM POSITION REQUIREMENTS:
· Requires medical degree in internal medicine or family practice.
· Board Certification in geriatric medicine is preferred.
· Must have current, valid license to practice medicine in the State of California and at least five years experience as a physician providing care to adults, including proven expertise in community-based long term care services for elders.
· Must be active member in good standing of the Medical Staff of SMCS and must also maintain accountability to the Department of Medicine, Internal Medicine Section of the Medical Staff, including credentialing standards and maintaining privileges at SMCS.
· Must have a strong knowledge base and clinical skills in geriatric medicine.
· Must have a working knowledge of the problems and issues affecting frail elders, including experience working with caregivers/families and community providers.
· Must have knowledge of effective utilization management and quality assurance activities.
· Special Skills/Equipment:
· Must be able to work effectively with a interdisciplinary team.
· Must have good oral and written communication skills.
· Must be able to prioritize and cope with emergent situations.
· Must have demonstrated a quality of practice that meets the highest standards.
· Must have BLCS for the Professional.
· Physical Requirements:
· Standing/walking 30%, sitting 60%, stooping, lifting 10%.
Must be able to respond to calls at any time of the day, 24 hours a day, seven days a week
