Overview: As part of an essential component of stroke services, this position is expected to provide professional nursing care for a disease specific subset of stroke patients. Stroke Coordinators are expected to provide individualized assistance to patients upon entering and being discharged from the health care system who have experienced an ischemic or hemorrhagic stroke. Under the direction of the Program manager and in collaboration with the multidisciplinary team, the Stroke coordinator will serve as a clinical resource in providing direct and indirect care for patients aimed at optimizing patient assessment, continuity of care, education to patients and family members and facilitate ongoing treatment. The Stroke Coordinator serves as a resource and contact for the program to facilitate the collaboration and coordination of patient care services to provide the highest level of care possible. Primary role is that of an expert nurse with essential education, consultation and researcher role. These duties and responsibilities are performed in a manner consistent with our mission, values and Mercy Service Standards.
- Develop, implement and facilitate care of the acute stroke patient. Respond to code stroke activations of both ER and inpatients. Facilitate and coordinate the treatment process based on established protocols and policies. Ensures that the acute stroke patient receives appropriate treatment based on patient condition within established benchmark times.
- Respond to pages from the ER/Units/Referring facilities. Facilitate and coordinate the transfer of acute stroke patients from other facilities.
- Communicates in a diplomatic manner and collaborates with physicians, physician assistants, nurse practitioners, nurses and ancillary departments to provide appropriate care for the stroke patient.
- Assess stroke patients daily, including NIHSS on patients that are discharging. Assures timely notification of changes in assessment to the appropriate provider.
- Assist with discharge planning, referrals to the neurology clinic, follow-up phone calls, and complete depression screening on stroke patients.
- Provide stroke education to patients and their families including but limited to diagnosis, warning signs of stroke, importance of activating EMS, personal risk factors, medications and follow-up care. Document stroke education in EHR. Meet the learning needs of patient/family.
- Assists with the development, implementation, and evaluation of clinical pathways from admission to discharge for the stroke patient. Monitor and review all charts daily for quality measure compliance. Assure co-workers are held accountable for following all policies and standards. Gives honest feedback in a timely fashion. Enthusiastic and able to be a positive influence to others. Role model to others, able to coach and mentor new staff.
- Assist with data management and collection. Documents data accurately in the utilization management module to ensure data for tracking and trending is accurate. Analyzes quality data to identify opportunities for improving practice. Formulates recommendations to improve practice and outcomes.
- Identifies potential risk management or clinical quality issues and resolves them through intervention or collaboration with other members of the healthcare team as appropriate or refers the issue to the appropriate individual(s).
- Maintains knowledge of and complies with current accrediting agency standards of care, State and Federal regulations, and third party payer requirements.
- Assist with patient feedback to EMS and referring facilities.
- Reviews daily readmissions and collaboratively creates a plan to positively impact desired outcomes.
- Participates in writing, reviewing and revising policies and protocols as needed.
- Educate staff on Stroke Center policies, protocols, processes and guidelines. Observes and identifies learning opportunities for co-workers. Communicates educational needs to unit Nurse Clinicians. Assists in the Orientation process for new co-workers.
- Functions as a clinical resource to co-workers.
- Accepts constructive feedback and makes changes appropriately. Meets deadlines and takes responsibility for accomplishing one’s own work.
- Participate in community outreach projects.
- Assist with Stroke Support Group.
- Active participant in Stroke Performance Improvement Team meetings and performance improvement activities.
- Actively seeks to improve skills and knowledge base for personal and job development through literature review and participation in in-services and other educational programs.
Performs other duties as assigned by the Stroke Program Manager and/or Stroke Medical Director.
Education: Graduate of an accredited School of Nursing, Bachelor of Nursing preferred
Licensure: Current License in the state of employment
Experience: 1 year acute care hospital setting, neurological care preferred
Certifications: Maintain BLS (CPR), NIHSS Stroke Scale Certification and at least 10 hours of stroke specific continuing education annually.
Other Skills & Knowledge: Strong assessment skills, time management skills and critical thinking skills. Advanced computer skills. Demonstrates skills in human relations and communication, written, verbal and electronic. Possesses the ability to assess data and assist personnel in providing appropriate level of care.
Working Conditions, Mental and Physical Requirements: Stand and walk most of the time on duty. Meet unit specific physical demands. Significant amount of bending and stooping, as well as lifting and turning patients with assistance. May be required to work differing schedules, shifts, days of the week or work frequency according to patient care requirements. High risk exposure to blood and body fluids.
